Diabetes Education

The Diabetes Self Management Training program provides education to those with Type 1, Type 2, and Gestational (Pregnancy) Diabetes

Topics include:

An Introduction to Diabetes
Nutrition – what should I eat?
Exercise
Medicines
Monitoring blood sugar – highs and lows
Immediate Problems
Long Term Problems
Problem-solving
Coping with the stress of diabetes

 

Are you at risk for diabetes?
Diabetes rates are much higher in Graham County than the state average. You may be at risk for diabetes depending on your health and family history. Find out if you are at risk by taking this quiz.

If you are at risk, look into how to prevent diabetes and see if you have symptoms. If you have any symptoms, make an appointment to see your doctor. A list of area providers is available if you do not have a regular doctor that you already see.

If you would like to participate in our Diabetes Education program, call us to set up an appointment. You will need a referral from your doctor in order to participate in the program. You can take this form with you to an appointment and get them to fax it to us.

After your referral is completed, the first step in the program is to complete an assessment. You can come to our office to complete this, or it can be done online. For your convenience, you can print of the assessment questions, answer what you can and bring them to your appointment.

Whooping cough (pertussis) is serious at any age, but islife-threatening to newborns and infants who have not completed the series of

vaccinations that would protect them from whooping cough. Each yearhundreds of infants are hospitalized for whooping cough.

Some infants die from it.
